About Your Visit
The initial appointment is an opportunity for us to get to know you, your concerns, and your goals. It’s also an opportunity for you to get to know our lovely staff and feel more at home. When you arrive, the front desk will collect any additional information we need and review your insurance details. You can save a great deal of time during the check-in by logging into your portal BEFORE your visit and filling out the section entitled New Patient. We strongly encourage all new patients to use the portal to complete the necessary forms, so that at your visit you can focus on your health – not the logistics. However, we do have tablets available at check-in for you to complete all of the new patient forms if you are not able to log into your portal beforehand.
Following the check-in, you’ll meet with one of our ENT specialists to discuss your symptoms and address your concerns. We encourage patients to bring a list of current medications and relevant medical records. Procedures, such as a scope exam of the nasal passages, or removal of ear wax, are often performed during the visit. You may also be sent for additional evaluations or scans, like blood work, hearing tests, or CT imaging. These help us gain a better understanding of your condition, so we can provide you with a tailored treatment plan.

Billing & Insurance
Insurance
Secure Online Payments
We accept assignment of benefits from many insurance companies. For those insurance companies we have a contract with, we will bill those plans and only require you to pay the co-payment at the time of service. If you have an insurance that requires authorization, it is ultimately your responsibility to obtain this from your Primary Care Physician. All charges that remain after 30 days will be charged a minimum fee of $10.00 per month, unless financing has been arranged. Any account that goes to collections will be charged a collection fee.
Co-Pays
All co-pays are due at check-in. However, if you are a NEW patient without insurance, you will be expected to bring $225.00 during your first visit, to be collected at check-in. Sometimes, that amount does not fully cover the visit – the cost depends on additional tests, procedures, or services.
Financial Policy
Minor Patients
Any patient under the age of 18 will not be seen without a parent or guardian present. The parent or guardian must accompany the minor throughout the visit to provide consent for care and to ensure accurate medical history is given. The policy will continue through future appointments, to keep treatment consistent and safe.
Appointment No-Show
We understand that sometimes unexpected events occur in all our lives. If you suddenly find that you will not be able to make your appointment, we encourage you to call as soon as possible, so we can help reschedule at a more convenient time. This also allows patients on our waiting list to have an
opportunity to be seen sooner, and helps all of our patients receive care as soon as possible. If you simply do not show up for your appointment and do not call to cancel it, there will be a $25.00 charge applied to your account.
Return Checks
There will be a $35.00 fee for all return checks. This fee covers the processing costs incurred by our practice any time that a check is returned. To avoid disruptions in your care, we ask that patients ensure sufficient funds are available before issuing a check.
